Important Information Regarding COVID-19
PPE Fee(Personal Protective Equipment)
Due to extraordinary circumstances resulting from the impacts of the COVID-19 pandemic, the cost of personal protective equipment (PPE) has dramatically increased. PPE may include items such as face masks and shields, gloves, gowns and shoe covers. Dentists and members of their teams use PPE to protect patients and themselves; new items are used for each patient.
Please note:
You may be charged for a portion of the increased cost of the personal protective equipment used for your appointment. As of the current market influx, our current fee is $10 per visit. This fee is subject to change should there be a PPE supply/demand issue in the future.
Patient Forms:
In an effort to reduce paper handling back and forth in the office, we have enabled our patient forms and questionnaire feature via our software. When confirming your appointments, you will receive a Covid-19 pre-screening assessment as well as our updated HIPAA consent form. Please complete, sign, and submit these forms prior to your arrival.
In recognition of ongoing news reports and the continued spread of the COVID-19 virus, we realize our patients have questions. As dedicated health care professionals, we want you to know that we remain as diligent as ever in protecting the health and safety of our patients and staff. Our practice strictly adheres to and exceeds the standards for infection control by wearing personal protective gear, using hospital-grade disinfectants, practicing the latest sterilization protocols, utilizing single-use disposable materials, and more. Every effort we have in place is to ensure the safety of our patients and staff. We want you to know that we are monitoring this situation very closely, and we will keep you updated regularly.
Symptoms of coronavirus are reported as flu-like, with varying degrees of severity. While we do not know everything about this virus to date, individuals who are elderly and/or immunocompromised appear to be the most at risk of a severe infection. If you have traveled to any areas within the past 14 days that are at high risk for contracting the coronavirus, please reschedule your appointment.
Symptoms may appear 2-14 days after exposure to the virus. To ensure your health and safety, and the health and safety of everyone at our practice, please reschedule your appointment if you or a member of your household are experiencing any of the following symptoms:
- Cough
- Fever or Chills
- Shortness of breath or difficulty breathing
- Fatigue
- Muscle or body aches
- Headache
- Recent loss of taste or smell
- Sore throat
- Congestion or runny nose
- Nausea, vomiting, diarrhea, or other flu-like symptoms
